Licensee Quarterly Reporting

Churches/Licensees, please use the following Excel spreadsheet to keep track of your music usage:

This form is for registered emPower Licensees to report music by emPower Publishers that they have used during the prior quarter.

Thank you for your reports -- they help us to provide the song writers and publishers the recognition and compensation they deserve.

Instructions

• Download the above Licensee Reporting Excel document to your computer, where you use it to keep track of your use of participating Publishers' songs.
• On the Excel document, you should enter each use of each composition on a new line. (i.e. we are looking to count total the number of uses of each composition during the quarter).   
• Participating publishers are listed at:    http://www.empowerma.com/musicrights/publisher/catalogs
• At the end of each calendar quarter, please complete and email this report to sue@empowerma.com   
• Due quarterly no later than April 14th, July 14th, October 14th, and January 14th for the prior quarter.
